The forms required for Short-Term Trips are included on this page for your convenience. If you need extra copies please open and print the appropriate document.
If you are traveling with a group organized through your local church, return the completed form to your team leader so they may include it in their package to Crossover. If there are any questions regarding your forms have your team leader contact the Short-Term Department at Crossover.
Those participating as individuals should return completed forms and communicate directly with Crossover.
